CDC Notice Regarding CDC Facilities COVID-19 Screening

| In order to protect the health of CDC/ATSDR employees and staff, all CDC staff must self-screen for symptoms or other risk factors of COVID-19 daily, using CDC’s COVID-19 screening tool on their mobile device or on a paper form at designated locations at facility entrances, prior to admittance to CDC facilities.

| Staff must use the tool to provide confirmation to the designated CDC representative that they are approved for access before being granted access to CDC facilities.

| CDC staff must answer the following questions in CDC’s COVID-19 screening tool. All responses should be “yes” or “no”
